Immunizations for School Attendance

If your child is going into kindergarten, 7th grade or not up-to-date on immunizations, please call the health department or your child's primary care doctor for an appointment.

Monday through Friday, 9 am to 11:45 am and 1 pm to 4 pm. On the first Tuesday of each month, they are open until 6 pm.
Be sure to bring any shot records you have for your child when you go to the clinic.

All immunizations and exemption affidavits must be up-to-date and current prior to starting school.
